Rather than spending your day inside one clinic, you'll work directly within the communities you serve, caring for patients with complex medical needs while collaborating closely with nurses, therapists, caregivers, social workers, and facility leadership. Your work helps reduce unnecessary hospitalizations, improve continuity of care, and provide proactive medical management that keeps seniors healthier, safer, and living with greater independence. What You'll Be Doing Perform comprehensive patient assessments and routine provider visits Diagnose and manage acute and chronic medical conditions Develop individualized treatment plans Coordinate care following hospitalization and rehabilitation Collaborate with physicians and interdisciplinary healthcare teams Educate patients, families, and caregivers Participate in quality improvement initiatives Help improve patient outcomes through proactive, preventative medicine Clinical Backgrounds That Translate Well Experience in any of the following areas is highly valued: Primary Care Family Practice Internal Medicine Geriatrics Long-Term Care Skilled Nursing Post-Acute Care Hospital Medicine Home Health Hospice & Palliative Care What We're Looking For Required Qualifications Active Minnesota Nurse Practitioner license Excellent assessment and diagnostic skills Ability to practice independently while collaborating with physicians Passion for serving older adults Reliable transportation and willingness to travel throughout Western Minnesota Preferred Qualifications Advanced wound care experience Chronic disease management expertise Community-based or senior care experience Wound Care Certification (WCC) is a plus Help Strengthen Healthcare Across Western Minnesota.
