WHAT DO WE DO?
CHSI is a private, non-profit organization established in 1973 to serve the health needs of migrant & seasonal farm workers and their family members. Today, CHSI has expanded its services and provides mental & physical healthcare to all individuals in our communities. Our services are provided on a more affordable sliding fee scale to those who are underinsured or uninsured. We also accept ND and MN Medicaid, Medicare, and most private insurance plans. We currently operate four nurse-managed clinics, and two victim advocacy program centers.
WHO ARE WE LOOKING FOR?
We are searching for a Per diem Mobile Health Provider (MD/DO ) to join our Moorhead, MN team. This is a per diem position with a flexible schedule based on mobile health operations and community need. Work is performed exclusively in mobile unit and community-based settings. Hours may include daytime mobile clinic coverage, outreach activities, and occasional evening, after-hours, and weekend events as needed.
Duties:
Provides primary care services to patients on CHSI mobile health units and outreach sites. May assist the Medical Director in the revision and implementation of practice principles, clinical protocols, and health care plans as needed. Consults with staff nurse practitioners and physician assistants as directed. Follows all applicable national patient safety goals in the provision of patient care. Serves as a role model for effective teamwork among staff and actively works to maintain positive morale and attitude. Leads the team in the team-based care model. Facilitates coordination and follow-up care between mobile health services and in-clinic providers to support continuity of patient care as needed. Provides care in collaboration with the center's team of physicians, nurse practitioners, behavioral health providers, nurses, and medical assistants. Serves as a resource person for clinical staff regarding patient care issues. Establishes and maintains rapport with community agencies. Perform duties are essential to the position and program. In addition to being a wonderful team member, we are seeking an individual with the below qualifications:
PHYSICAL ABILITIES
In an eight hour workday, the employee must be able to: bend and crouch occasionally, sit continuously and stand or walk frequently. Employee must be able to lift up to 50 pounds occasionally. Employee must be able to use hands/arms, feet/legs for repetitive fine to large motor skill movements.
Qualifications:
Graduate of an accredited Doctor of Medicine or Doctor of Osteopathic Medicine program. Minimum of 2 years of experience in community health or primary care setting
Knowledge/Skills/Abilities:
Ability to provide effective care consistent with medical best practices. Ability to see all ages of patients, including pediatrics and geriatrics. Ability to provide prenatal care in an outpatient setting. Ability to perform women's health procedures including LARC placement, Endometrial biopsy, and possibly colposcopy. Ability to provide healthcare in an unbiased manner to patients of many cultural backgrounds. Ability to work independently.
Licensure and Credentials:
Licensure and certification as required by the discipline.
