Provides help with all aspects of patient care, including diagnosis, treatments and consultations as part of a physician/nurse team. Works within a Specialty Care team, in both inpatient and outpatient situations and can perform independently or as part of the treatment team ensuring continuity of patient care in all settings. Job Description:
Principle Responsibilities Provides clinical care for patients Completes comprehensive patient assessments. Determines diagnosis and treatment of illness. Orders and interprets medical diagnostics in tandem with attending physician. Prescribes medications, treatments, therapy and medical equipment. Conducts patient education when working to manage chronic illness or wellness care. Refers for specialty care when determined. Consults with physician when patient needs outside of Nurse Practitioner scope of practice, diagnosis or treatment plan is unclear. Participates in health care team Promotes communication within the team and other interdisciplinary health care staff. Acts as resource to members of the team. Collaborates with hospital staff by consulting and rounding on patients. Works with primary care team for program development. Participates in research activities when opportunity available. Acts as liaison with outside agencies as needed. Other duties as assigned. Job Requirements Master's degree Completion of accredited Nurse Practitioner or Physician Assistant program. required Minimum 2 years of Integrative Medicine or Functional Medicine Experience Minimum of 2 years of experience providing Integrative Medicine or Functional Medicine consultations. Licensed Registered Nurse- MN Board of Nursing required upon hire Certified Nurse Practitioner

Pay Range Pay Range:
$56.48 to $78.15 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
