Primary Care / Family Medicine Position Summary The Family Nurse Practitioner (FNP) provides comprehensive, patient-centered primary healthcare services to individuals and families across the lifespan. The FNP assesses, diagnoses, treats, and manages acute and chronic health conditions while emphasizing disease prevention, health promotion, and patient education. Working collaboratively with physicians, nurses, and other healthcare professionals, the FNP delivers high-quality, evidence-based care that improves patient outcomes and supports continuity of care. Essential Duties and Responsibilities Perform comprehensive health assessments, including obtaining medical histories and conducting physical examinations. Diagnose and manage acute and chronic illnesses across all age groups. Order, interpret, and evaluate diagnostic tests, laboratory studies, and imaging results. Develop and implement individualized treatment plans based on clinical findings and evidence-based practice guidelines. Prescribe medications and therapies in accordance with state regulations and organizational policies. Provide preventive healthcare services, including routine screenings, immunizations, and wellness examinations. Educate patients and families regarding disease processes, treatment options, medication management, lifestyle modifications, and preventive health measures. Monitor patient progress and modify treatment plans as clinically indicated. Coordinate care with physicians, specialists, hospitals, community resources, and ancillary healthcare providers. Document patient encounters accurately and timely within the electronic health record (EHR). Participate in quality improvement initiatives, patient safety activities, and performance improvement programs. Maintain compliance with federal, state, and organizational regulations, policies, and standards of care. Promote patient satisfaction through compassionate, culturally competent, and patient-centered care. QualificationsEducation Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited nurse practitioner program. Graduate of an accredited Family Nurse Practitioner program. Licensure and Certification Current, unrestricted Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) licensure. Current Family Nurse Practitioner certification through the American Academy of Nurse Practitioners Certification Board (AANPCB) or American Nurses Credentialing Center (ANCC). Current DEA registration and state prescriptive authority (as applicable). Current Basic Life Support (BLS) certification; Advanced Cardiovascular Life Support (ACLS) preferred. Knowledge, Skills, and Abilities Strong clinical assessment, diagnostic, and treatment skills. Knowledge of primary care, preventive medicine, and chronic disease management. Ability to interpret diagnostic and laboratory findings accurately. Excellent communication, interpersonal, and patient education skills. Proficiency in electronic health record (EHR) systems. Ability to work independently and collaboratively within a multidisciplinary healthcare team. Strong organizational, critical thinking, and problem-solving abilities. Commitment to maintaining patient confidentiality and professional ethical standards. Preferred Experience One to three years of experience as a Family Nurse Practitioner in a primary care, family medicine, internal medicine, or community health setting. Experience managing patients with multiple chronic conditions, preventive care programs, and population health initiatives. Pay:
