I. JOB SUMMARY
The Provider Services Nurse Practitioner (NP) at Big Sky Home Health & Hospice plays a vital role in supporting the organization's mission of providing life-changing service by helping patients receive primary care services as they navigate a complex medical system. This role focuses on clinical service delivery, provider support, care coordination, and operational excellence across Big Sky's care continuum. The Provider Services NP collaborates closely with the Executive Director, clinical leadership, and interdisciplinary teams to ensure high-quality, cost-effective, timely, and compliant care delivery. All services are provided within the NP's scope of practice and aligned with organizational standards, regulatory requirements, and best practices.
II. DUTIES & RESPONSIBILITIES
Under the supervision of the Executive Director, provide clinical support and services that promote safe, effective, and efficient healthcare delivery across Big Sky's service lines as a primary care physician. Support operational clinical workflows, provider services initiatives, and care coordination efforts to ensure continuity and quality of care. Deliver clinical services as appropriate via in-person visits, telephone, or telehealth encounters across multiple care settings, including residential, facility-based, and home environments. Serve as a clinical resource to interdisciplinary teams, supporting evidence-based practice, quality improvement initiatives, and best-practice standards. Perform comprehensive patient evaluations including history taking, assessment, diagnosis, treatment planning, prescribing medications, patient education, and evaluation of care outcomes. Collaborate with physicians, advanced practice providers, nursing staff, care coordinators, and operational leaders to optimize patient outcomes and service delivery. Ensure compliance with state-specific standards of practice for Nurse Practitioners and all applicable federal, state, and local regulations. Document clinical findings, assessments, and care plans accurately, thoroughly, and timely in the electronic medical record. Maintain strict patient confidentiality and safeguard protected health information at all times. Participate in provider services initiatives, clinical protocol development, and quality assurance activities as assigned. Maintain a safe working environment for self, patients, families, and staff by following infection control standards and utilizing appropriate personal protective equipment (PPE). Engage collaboratively with internal teams and external partners including healthcare providers, community resources, patients, and family members. Represent Big Sky Home Health & Hospice professionally at all times, fostering a cooperative and respectful work environment through tact, diplomacy, and a positive attitude. Demonstrate professionalism by being reliable, autonomous, accountable, and adaptable to changing clinical and operational needs. Manage workload effectively, prioritize responsibilities appropriately, and utilize work time productively. Maintain humility, openness to feedback, and willingness to accept coaching, constructive criticism, or corrective action when necessary.

III. PRODUCTIVITY & PERFORMANCE EXPECTATIONS
Meet established productivity, quality, and operational performance standards as defined by Provider Services leadership and the Executive Director. Productivity expectations may vary based on encounter complexity, care coordination activities, quality initiatives, and non-visit-based provider services responsibilities. Complete all clinical documentation accurately and within 7 days of each patient encounter or service provided. Participate in on-call or clinical support rotation as required to meet organizational needs.
IV. JOB REQUIREMENTS
Master's degree from an accredited Nurse Practitioner program with current and unrestricted state licensure, prescriptive authority, and credentialing eligibility. Ability to successfully pass background check and drug screening. Maintain a valid driver's license, access to reliable transportation, and willingness to travel as needed. Fluency in reading, writing, and speaking English. Strong verbal and written communication skills with the ability to communicate effectively with providers, operational leaders, patients, families, and community partners. Working knowledge of applicable federal and state regulations including HIPAA, OSHA, and healthcare compliance standards.
Clinical Competencies:

Comprehensive Assessment:
  • Ability to perform a thorough assessment considering physical, cognitive, and psychosocial aspects.
  • Ability to assess and diagnose acute and chronic illnesses and identify the risk factors associated with these conditions.
  • Proficiency in evaluating functional status including activities of daily living (ADLs) and instrumental activities of daily living (IADLs).

Chronic Disease Management:
  • Expertise in managing common chronic conditions in the adult and elderly populations (e.g., diabetes, hypertension, osteoarthritis).
  • Skill in developing and implementing personalized care plans for chronic disease management.
  • Work closely with MA / support staff to coordinate patient care.
Delegate tasks as appropriate. 3.
Medication Management:
  • Knowledge of geriatric pharmacology.
  • Proficiency in medication reconciliation and management, considering potential interactions and side effects.

Palliative and End-of-Life Care:
  • Competency in providing palliative care and managing end-of-life issues.
  • Ability to facilitate discussions about advance care planning.

Fall Risk Assessment and Prevention:
  • Skill in assessing and addressing fall risk factors.
  • Knowledge of strategies to prevent falls in the elderly.

Cognitive Function Assessment:
  • Ability to conduct cognitive assessments (e.g., Mini-Mental State Examination).
  • Competency in identifying and managing cognitive impairments.
Communication and Patient Education:

  • Strong communication skills to interact with patients, their families, and other healthcare professionals.
  • Ability to explain complex medical concepts in a clear and understandable manner.

  • Sensitivity to cultural differences and the ability to provide culturally competent care.
  • Recognition of the importance of family dynamics in the care of geriatric patients.

Patient and Family Education:
  • Capability to educate patients and their families about health conditions, medications, and self-care.
  • Ability to promote health literacy and empower patients to actively participate in their care.
Home Care Competencies:

  • Skill in assessing the patient's home environment for safety and making recommendations for improvements.

  • Competency in using telehealth technologies for remote patient monitoring and virtual consultations.
