Liberty Healthcare seeks a dedicated and skilled Nurse Practitioner to join our team, contributing to our mission of providing innovative and high-quality medical services. In this role, you will utilize a patient-centered approach to manage and coordinate patient care, focusing on prevention, wellness, and the management of chronic conditions. Responsibilities Conduct comprehensive patient assessments, including evaluating patient history and physical examinations. Develop and manage treatment plans based on scientific rationale, standards of care, and professional practice guidelines. Prescribe medications following the state's prescribing guidelines. Educate and counsel patients and their families on preventive care, medical issues, and use of prescribed medical treatments and/or medications. Monitor the effectiveness of interventions and make necessary adjustments in care plans. Collaborate with healthcare professionals to ensure continuity of patient care. Maintain patient records and ensure confidentiality and compliance with all applicable laws and regulations. Required Skills Strong clinical skills with the ability to assess and diagnose patients. Proficiency in managing a broad spectrum of cases with multiple co-morbidities. Ability to communicate clearly and effectively with patients and team members.
