  • No Nights, Weekends, or On-Call afterhours/weekends/Holidays About the Role We are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our post-acute care team serving skilled nursing facilities (SNFs) and long-term care (LTC) patients.
In this role, you will work in direct supervision with two established post-acute care physicians who have over 30 years of combined experience in this setting, providing patient-centered medical management for individuals recovering after hospitalization or needing long-term care services. What You'll Do Perform new patient admissions , histories, and physical exams. Manage acute care and chronic medical conditions in collaboration with physicians. Order and interpret lab work, imaging, and other diagnostics; prescribe medications per licensure. Provide wound care , pain management , palliative care , and preventive health education . Collaborate with nursing staff , therapy teams , and families to ensure safe care transitions. Maintain accurate and timely documentation in the electronic health record (EHR) . National certification (Adult, Family, Acute Care, or Geriatrics preferred). DEA registration (if applicable). 1-2 years' experience in post-acute care , skilled nursing , hospitalist medicine , geriatrics , or rehabilitation preferred. Strong clinical assessment, communication, and teamwork skills. Work Environment Practice in skilled nursing facilities and long-term care settings .
