This is a mobile, facility-based primary care position — not a traditional clinic job. You'll care for patients where they live, work closely with facility nursing teams and families, and have the independence to manage your own patient population with the support of CHP's clinical and administrative team. Defined facility territory along the I-80 corridor (Facilities are in Aurora/Grand Island/Harvard/Northwest Lincoln) No traditional clinic schedule Autonomous practice with clinical and administrative support Opportunity to add wound care to your clinical skill set CHP will pay for wound care certification for the right candidate What Your Week Looks Like You'll begin your scheduled day at 8:00 AM and travel to established CHP partner facilities located along the I-80 corridor between Lincoln and Grand Island, including Aurora, Harvard, Grand Island, and Northwest Lincoln. Your first facility arrival will typically be around 9:00-9:30 AM depending on the day's location. Travel is built into your workday, and mileage/trip compensation is paid in addition to your salary. Rather than seeing patients in a traditional office setting, you'll round within senior living and skilled nursing communities and develop ongoing relationships with the residents you care for. What You'll Do As a CHP Nurse Practitioner, you'll independently provide comprehensive primary and geriatric care while collaborating closely with facility staff, families, specialists, and the CHP team.

Responsibilities include:

Assess, diagnose, and manage acute and chronic medical conditions Provide ongoing primary care for medically complex older adults Perform comprehensive assessments and follow-up visits Order and interpret laboratory and diagnostic testing Prescribe and manage medications and individualized treatment plans Coordinate care with nursing staff, specialists, families, and other members of the care team Educate patients and families regarding treatment and ongoing care Document visits accurately and timely in eClinicalWorks Provide wound assessment and management after appropriate training/credentialing What We're Looking For We're looking for an experienced Nurse Practitioner who enjoys independence, is comfortable making clinical decisions, and wants to build meaningful relationships with patients rather than move through a traditional high-volume clinic schedule.

Required:

Current Nebraska APRN/Nurse Practitioner license Primary care experience Strong assessment, diagnostic, and clinical decision-making skills Ability to practice independently in facility-based settings Excellent communication and collaboration skills Reliable transportation and comfort traveling within the assigned territory Wound care experience is a plus — but it is not required.
