Full-Time or Part Time Option Make a Meaningful Difference in the Lives of Older Adults Community Healthcare Partners (CHP) is seeking a compassionate, experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing behavioral health team. This is an excellent opportunity for a provider who is passionate about caring for the geriatric population and enjoys working in a collaborative, patient-centered environment. As a PMHNP with CHP, you will provide psychiatric evaluations, medication management, and ongoing behavioral health care for patients in skilled nursing facilities and assisted living communities. You'll work alongside primary care providers, therapists, nurses, and support staff to deliver comprehensive, high-quality care that truly improves patients' quality of life. Our patients commonly experience conditions such as: Dementia Depression Anxiety PTSD Other age-related behavioral health disorders Your expertise will have a direct and lasting impact on helping patients live healthier, more fulfilling lives. What You'll Do Perform comprehensive psychiatric evaluations and assessments Diagnose and treat a variety of psychiatric and behavioral health conditions Prescribe, adjust, and monitor psychiatric medications Order and interpret appropriate diagnostic testing when indicated Develop individualized treatment plans Educate patients, families, and caregivers regarding diagnoses and treatment options Maintain accurate and timely electronic medical records Collaborate closely with therapists, primary care providers, the Medical Director, supervising psychiatrist, Triage Nurse, and office staff Participate in quality improvement initiatives and continuing professional development Practice Environment Patients are seen in skilled nursing and assisted living facilities throughout the Lincoln, Omaha, and Fremont metro areas Practice under a collaborative Practice Agreement with a supervising psychiatrist Dedicated office staff manages insurance authorizations, and administrative support so you can focus on patient care Schedule Flexible Full-Time Schedule Five 8-hour shifts per week No evenings No weekends Typical schedule: 2 Days

• Fremont Compensation & Benefits Competitive base salary for FT Staff Unlimited productivity bonus structure Average providers seeing 15+ patients per day can expect annual earnings of $120,000
• 200,000+ (Full Time Staff) No cap on productivity incentives

Part Time Rate:

$85/hour Flexible scheduling Collaborative, supportive team environment Opportunity to build a rewarding long-term practice Start date is contingent upon completion of credentialing. Qualifications Master's or Doctorate Degree in Nursing with Psychiatric/Mental Health specialization Graduate of an accredited Psychiatric Mental Health Nurse Practitioner program Current Nebraska Nurse Practitioner license Minimum of 1-2 years of psychiatric care experience required Experience in behavioral health and/or primary care preferred Excellent communication and interpersonal skills Strong clinical judgment and organizational abilities Passion for providing compassionate, patient-centered care Why Community Healthcare Partners?
