The Advanced Practice Provider Primary Care will report within a supportive ambulatory leadership structure—typically to a Medical Director of Primary Care with practice management oversight—while collaborating closely with attending physicians, RN care coordinators, behavioral health, pharmacy, PT/OT, social work, and on-site specialty consults; the Advanced Practice Provider Family Medicine scope includes same-day acute visits, chronic disease optimization, annual wellness and preventive screenings, care gap closure, population health initiatives, and quality improvement projects, all streamlined by protocol-driven order sets, clinical decision support, and an efficient EHR; friendly nurses with a 5-star service mindset and a diverse team of medical assistants, scribes, and front-desk professionals help the Advanced Practice Provider Primary Care stay patient-facing, productive, and fulfilled.
