As a Nurse Practitioner, you will work closely with staff and community partners to prevent, promote and protect the health of the citizens of the Greater Nashua Region through community outreach, education, collaboration and health services This position supports the community health department through providing patient care as an advanced practice registered nurse (APRN). This position coordinates the hepatitis C (HCV) program, including treatment and case management of clients. The APRN works closely with the medical director, clinical and support staff to provide care primarily surrounding communicable disease, with a focus on sexual wellness and immunization programs, on a part-time basis. This is a part-time 20 hours per week Unaffiliated position, with a varying work schedule, and provides an excellent work life balance. The position is grade 18, hourly rate dependent upon experience.SKILLS/QUALIFICATIONS NH
Advanced Practice Registered Nurse (APRN) license Bilingual in Spanish and/or Portuguese preferred Knowledge of substance misuse disorder and mental health challenges Knowledge of sexual health medicine including hepatitis C Care of clients who are experiencing homelessness, substance misuse mental health issues. Public Health Initiatives Knowledge of Microsoft Office, Google, and general technology literacy Ability to recognize and manage one's own behaviors, moods, and impulses Ability to create an inclusive, equitable, and welcoming environment for all individuals Efficiently train to competency with EMR and multiple databases Communicate through high-level written and verbal skills Work effectively in an interdisciplinary, team-focused environmentHOW TO APPLY
