Competitive Salary + Bonus Opportunities About the Role TruCare Management is seeking a full-time Nurse Practitioner who wants to become an integral part of a skilled nursing facility and make a real difference in the lives of residents every day. This is not a typical provider role. We are looking for someone who enjoys building relationships, collaborating with facility staff, and becoming a trusted clinical presence throughout the building. The ideal candidate will round daily, provide primary care services to residents, assist nursing staff with clinical concerns, and help improve overall resident outcomes. Perform initial assessments and routine follow-up visits. Manage chronic and acute medical conditions. Round throughout the facility daily and maintain a visible clinical presence. Collaborate closely with nursing staff, therapists, social workers, and administration. Respond to resident condition changes and provide timely interventions. Participate in care plan meetings and interdisciplinary discussions. Communicate with residents and families regarding care plans and medical concerns. Help reduce avoidable hospitalizations through proactive clinical management. Maintain accurate and compliant medical documentation. Support facility quality improvement and regulatory readiness initiatives. What We're Looking For Required Qualifications Active Nurse Practitioner license in the state of practice. Board Certified Nurse Practitioner (FNP, AGNP, ANP, or equivalent). Strong clinical assessment and critical thinking skills. Excellent communication and relationship-building abilities. Ability to work independently while collaborating effectively with facility teams. Preferred Qualifications Experience in Skilled Nursing Facilities (SNF), Long-Term Care (LTC), Assisted Living, or Geriatrics. Experience caring for medically complex patients.
