09/03/2026 Job Description Job Description Job Description Elmora Family Practice is currently seeking for a professional Bilingual Spanish & English Nurse Practitioner who can deliver patient-centered Primary Medical Care to a wide variety of patients in a community setting. The Provider will be responsible in caring for patients, maintaining accurate and current patient records, and administering follow-up to patients as required. The successful candidate will work alongside our current APN and MD. As a team we will continue to deliver premium care to each and every patient in our community.
Advanced Nurse Practitioner responsibilities:
Practice in accordance with the state's laws, rules and regulations. Practice as a member of an interdisciplinary health care team for all patients (13 and older). Provide primary health care through direct care, consultation, collaboration, coordination and/or referral. Assess and treat patients for a variety of chronic and acute conditions Review any and all results from ordered testing Update patient records and check records for accuracy at each patient appointment Perform initial patient screenings and patient history and physical examinations Appointments scheduled 5 days a week;
Weekends and Major Holidays Off Advanced Nurse Practitioner Qualifications:
Bilingual Spanish & English (Mandatory) Valid Nurse Practitioner / Physician Assistant license Must have exceptional bedside manner and confidence 4+ years clinical experience Must maintain current license and certification during duration of employment Ability to work within a team and take direction Strong written and interpersonal communication skills Experience using EHR/EMR
