Job Description
Job Summary We are seeking a dynamic and compassionate Nurse Practitioner - Primary Care to join our dedicated healthcare team. In this vital role, you will provide comprehensive primary care services, including health assessments, diagnosis, treatment, and management of a diverse patient population across various age groups. Your expertise will support patient-centered care initiatives, promote health education, and collaborate closely with physicians and other healthcare professionals to ensure optimal patient outcomes. This position offers an exciting opportunity to make a meaningful difference in community health through proactive and innovative care delivery. Responsibilities Conduct thorough physical examinations and health assessments for patients of all ages, utilizing advanced clinical skills and physiology knowledge. Prescribe medications, administer injections, perform IV infusions, and manage acute pain using evidence-based practices while adhering to DEA regulations. Utilize EMR/EHR systems such as Epic, eClinicalWorks, or Cerner for accurate medical documentation, medical records management, and patient data entry. Collaborate effectively with healthcare professionals across disciplines to develop personalized care plans, including discharge planning and case management strategies. Perform diagnostic evaluations such as sonography, spirometry, or laboratory specimen collection/processing to support accurate diagnosis and treatment. Provide patient education on health coaching topics including nutrition, disease prevention, medication administration, and chronic disease management like geriatrics or hospice & palliative medicine. Participate in telehealth consultations and triage services to deliver accessible care while maintaining HIPAA compliance. Requirements Valid Nurse Practitioner license with current state certification; experience working with children, geriatrics, or individuals with disabilities is preferred. Demonstrated proficiency in EMR/EHR systems such as Epic, Athenahealth, or Cerner; experience with medical documentation and coding (CPT, ICD-10/ICD-9) is essential. Critical care experience including ICU or Level I/II trauma center exposure; hospital medicine or emergency room experience highly valued. Strong clinical skills in procedures such as suturing, catheterization, spinal taps, dermal fillers or esthetic laser treatments are a plus. Knowledge of Workers' Compensation law and outpatient/inpatient care protocols; experience in behavioral health or mental health settings is advantageous. Excellent client communication skills with the ability to perform comprehensive patient assessments and physical examinations confidently. Ability to work collaboratively within a multidisciplinary team in fast-paced environments like urgent care centers or assisted living facilities.
