UHP is currently affiliated with 182 providers consisting of 119 physicians and 63 advanced practice providers operating out of 54 locations across Southern, Central, and Northern California.POSITION SUMMARY
The Nurse Practitioner/Physician Assistant provides comprehensive patient care through the evaluation, management, and treatment of patients. Under the direction of a collaborating physician, this role works alongside other members of the healthcare team to deliver high-quality, patient-centered care in accordance with established protocols and scope of practice. What you will be doing Evaluate patients by reviewing medical history and performing appropriate physical exams in a medical office or clinical setting. Interpret and evaluate patient data as authorized by physician for the purpose of determining management and treatment. Consult with physician as appropriate. Assess, diagnose, and manage patient health conditions. Prescribe, recommend, and regulate appropriate pharmacologic medicine. Provide patient education and counseling regarding conditions, treatment plans, and health maintenance. Collaborate with physicians and other members of the healthcare team to provide optimal patient care. Maintain current knowledge of clinical practices and options to support positive patient outcomes. Document appropriately and communicate with supervising physicians in a timely manner, including evaluations, treatment plans, progress reports, treatment notes, and other reports as requested. Provide care for Advanced Prostate Cancer (APC) patients Prescribed and manage advanced therapeutics Conduct peer-to-peer reviews forPSMA PET
imaging and APC medications Collaborate with Medical Oncology, Radiation Oncology and Nurse Navigation Screen patients for Advanced Therapies and Clinical Trial Eligibility What you will bring to the team Current NP or PA-C license Master's degree in Nursing/Physician Assistant program required. Current BLS certification required. Minimum of 2-5 years' experience as a mid-level provider preferred. Strong communication and interpersonal skills with patients regarding condition, prognosis, health needs, and available services. Proficiency of Electronic Medical Record (EMR) systems. Knowledge in Microsoft Office products (Word, Excel, PowerPoint, Teams). Knowledge of HIPAA compliance rules and regulations. Bilingual language skills a plus.Travel Requirements:
