09/03/2026 Job Description Job Description Nurse Practitioner Job Overview We are seeking a dedicated and skilled Nurse Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care, perform assessments, and develop treatment plans in various medical settings. This role requires a strong background in clinical practice, excellent communication skills, and the ability to work collaboratively with other healthcare professionals. Responsibilities Primary care office, home visits, Assisted Living facility visits. Conduct comprehensive physical examinations and patient assessments. Develop and implement individualized treatment plans based on patient needs. Provide patient education on health management and disease prevention. Collaborate with physicians and other healthcare providers to ensure coordinated care. Maintain accurate medical documentation in compliance with HIPAA regulations. Manage acute and chronic conditions across various specialties including internal medicine, geriatrics, and urgent care. Skills The successful candidate will possess a combination of the following skills: Proficiency in EMR/EHR systems such as Tebra. New grads are welcome to apply Knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), CPT coding Ability to perform diagnostic evaluations and manage complex patient cases including those requiring telehealth services or home care support. Competence in patient care techniques and physical examinations.
