Job Description
Overview We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. This role offers an exciting opportunity to deliver comprehensive primary care, acute and chronic disease management, and preventive health services across diverse patient populations. The ideal candidate will possess a strong clinical background, excellent communication skills, and a passion for improving patient outcomes through collaborative practice and innovative care delivery. As an integral part of our healthcare community, you will utilize Athena Health EMR/EHR systems, engage in patient education, and work closely with other healthcare professionals to ensure exceptional care. Duties Conduct thorough physical examinations and patient assessments to diagnose health conditions accurately. Develop and implement personalized treatment plans, including prescribing medications in compliance with DEA regulations. Manage a wide range of medical issues such as acute pain, infectious diseases, geriatrics, pediatrics, behavioral health, and chronic illnesses. Collaborate with physicians, specialists, and other healthcare providers to coordinate patient care and ensure seamless service delivery. Utilize EMR/EHR for accurate medical documentation and record keeping. Engage in patient education on topics like nutrition, family planning, memory care, hospice & palliative medicine, and health coaching to promote wellness. Provide triage services via telehealth or in-person consultations to prioritize urgent cases effectively. Experience Valid Family Nurse Practitioner certification. Hands-on experience with medical office procedures including CPT coding and ICD coding for accurate billing and documentation. Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks is essential. Background in acute care environments including hospital inpatient units or urgent care clinics; experience with trauma or neurocritical care is a plus. Knowledge of infectious disease protocols and infection control practices; experience working with individuals with disabilities or developmental delays is advantageous. Strong understanding of healthcare diagnosis processes involving physiology knowledge and diagnostic evaluation skills. Experience working within collaborative practice agreements under nurse practitioner scope of practice laws; familiarity with HIPAA regulations is required.
