As part of this mission, we are seeking a skilled Family Nurse Practitioner (FNP) to join our dedicated team of healthcare professionals. This role will primarily involve providing comprehensive primary care services, chronic disease management, and preventative care across all ages, genders, diseases, and parts of the body. With your expertise, we aim to bridge the gap between the demand for high-quality health care and the community's needs, ensuring effective health outcomes and patient satisfaction. Responsibilities Conduct comprehensive patient assessments, diagnose health conditions, develop and manage treatment plans. Prescribe medications and therapies, in adherence to evidence-based standards. Provide health education and counselling to patients and their families on preventive health, nutritional plans, and lifestyle changes. Collaborate with other healthcare professionals to enhance clinical practice and patient health outcomes. Maintain detailed patient records and prepare reports to ensure effective management of care plans. Stay updated with advancements in healthcare and integrate best practices into patient care. Required Skills Expert knowledge in primary care and family medicine. Strong diagnostic and clinical decision-making skills. Excellent communication and counseling abilities. Proficient in using healthcare IT systems and electronic health records.
