2 days a week varied start and end time between the hours of 7am-5pm.

Summary:

Provides medical care in support of professional medical staff. Provides a variety of medical services to patients in accordance with established laws and procedures. Works cooperatively with other health professionals under the direct supervision of a physician.

Responsibilities:

EDUCATION

Graduate from an accredited school of nursing Maintains a current New York State Professional RN license Completion of an accredited program for Nurse Practitioners Maintains Adult, Acute Care or Family Nurse Practitioner Certification Master's degree preferred

EXPERIENCE

Three (3) years' experience as a Nurse Practitioner preferred Experience in family practice/specialty setting preferred

KNOWLEDGE, SKILLS AND ABILITY

Certification in CPR and annual re-certification required ACLS certification preferred Knowledge of Patient Centered Medical Home, with

MIPS, HEDIS
