Nurse Practitioner/Physician Assistant (NP/PA) Community Care Physicians - Community Care Family Practice Colonie Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join Community Care Family Practice Colonie , a well-established primary care practice providing comprehensive care to patients of all ages. Position Overview As an Advanced Practitioner at Community Care Family Practice Colonie, you will deliver full-spectrum family medicine services in a collaborative, team-based environment. You will practice to the full extent of your license with strong physician support, integrated clinical services, and scheduling flexibility designed to promote work-life balance and long-term professional satisfaction. Responsibilities Conduct comprehensive patient evaluations and manage acute and chronic conditions. Order and interpret diagnostic studies, prescribe medications, and coordinate referrals as needed. Provide preventive care, health maintenance, and patient education. Collaborate closely with physicians, nurses, behavioral health consultants, and support staff to ensure continuity of care. Maintain accurate, timely clinical documentation in accordance with organizational standards. Practice Highlights Flexible scheduling options designed to support work-life balance, including built-in administrative time for Full time Advanced Practitioners to support charting and care coordination. On-site behavioral health consultant available multiple days per week, providing outpatient therapy and serving as a clinical resource for providers. Recently renovated facility with private provider offices and dedicated exam rooms, supporting efficient workflows and a professional clinical environment. Compensation & Professional Development Competitive salary starting at $125,000 and comprehensive benefits package. License and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.
