Nurse Practitioner (NP)

Long Island Care Center is seeking a qualified Nurse Practitioner to join their clinical team in a skilled nursing and post-acute care setting. Bilingual- Chinese (Mandarin and/or Cantonese speaker preferred)

Description:

The NP will provide comprehensive primary care, manage chronic conditions, and perform acute assessments to minimize hospital readmissions.

Key Responsibilities:

Conduct comprehensive physical exams and assessments of sub-acute and long-term care residents. Collaborate with nursing staff, physicians, and multidisciplinary teams to develop and implement care plans. Manage acute and chronic illnesses, including ordering diagnostic tests, prescribing medications, and ordering treatments. Provide timely documentation within the EMR system.

Qualifications:

Active, unencumbered Nurse Practitioner license (FNP, AGNP, or ANP). Prior experience in skilled nursing, post-acute care, or geriatric care preferred. Strong communication and collaboration skills.
