15 completed visits per month About the Role We are seeking compassionate and experienced Nurse Practitioners and Physician Assistants to conduct in-home Annual Preventive Health Risk Assessments for Medicare, Medicaid, and Affordable Care Act (ACA) members. These assessments are provided at no cost to members and are completed within the member's home or personal living environment. This role offers flexible scheduling with appointments typically lasting 60 to 75 minutes. Providers play a critical role in identifying health risks, documenting clinical findings, and supporting improved member outcomes.
Travel Requirement:
Candidates must be willing to travel within approximately a 50-minute radius of their assigned service area.
Credentialing Requirement:
Providers must be credentialed with Excellus, New York State Medicare, and Medicaid to prevent delays in onboarding and service delivery. Compensation & Benefits Competitive market-rate compensation per completed assessment Mileage reimbursement at the current IRS rate after the first 30 miles driven each day Flexible scheduling with daytime and evening availability Key Responsibilities Conduct comprehensive in-home Annual Preventive Health Risk Assessments. Deliver high-quality, member-focused care while meeting health plan quality and satisfaction standards. Accurately document assessment findings and submit completed chart notes within 24 hours of each visit. Respond to charting inquiries and documentation requests within 48 hours. Maintain regular communication with clinical leadership and support teams. Participate in required onboarding, training programs, meetings, and continuing education activities. Ensure compliance with organizational policies, clinical guidelines, and regulatory requirements, including HIPAA standards. Maintain professionalism and sensitivity while providing care within members' homes. Minimum QualificationsEducation Nurse Practitioner Master's or Doctoral degree from an accredited Nurse Practitioner program (FNP, AGNP, ANP, or equivalent). Current and unrestricted Nurse Practitioner license. Physician Assistant Master's degree from an accredited Physician Assistant program. Current and unrestricted Physician Assistant license. Experience Home health experience within the past five years (required). One or more years of in-home assessment experience (strongly preferred). Required Skills & Competencies Proficiency with electronic medical records (EMR) and general computer applications. Strong clinical assessment and documentation skills. Knowledge of CMS risk adjustment methodologies and Affordable Care Act requirements. Ability to communicate effectively with members, caregivers, providers, and interdisciplinary teams. Strong critical thinking and clinical decision-making abilities. Ability to work independently while maintaining high standards of quality and accuracy. Adaptability in a dynamic healthcare environment. Commitment to patient confidentiality and HIPAA compliance. Excellent interpersonal skills with a collaborative, member-centered approach. Ability to proactively identify and resolve member, provider, and health plan concerns. Physical Requirements Ability to enter and work within members' homes and personal living environments. Ability to lift up to 20 pounds. Ability to bend, kneel, and remain in various positions for extended periods as needed during assessments. Performance Expectations To be successful in this role, providers must: Complete all required self-paced and instructor-led training within established timelines. Make available a minimum of 15 appointment slots per month. Consistently meet documentation timeliness and quality standards. Maintain strong member satisfaction scores and clinical quality outcomes. Demonstrate reliable communication and responsiveness with leadership and clinical support teams.
