What You'll Do:
As a key clinical leader, you will provide comprehensive primary care services, including diagnosis, treatment, prevention, and care coordination. You'll work closely with physicians and interdisciplinary team members to ensure high-quality, patient-centered care.
  • Perform patient assessments, histories, and physical exams
  • Diagnose conditions and develop individualized treatment plans
  • Prescribe medications in accordance with NYS guidelines
  • Perform minor procedures and manage chronic conditions
  • Review labs, imaging, and patient communications with timely follow-up
  • Coordinate referrals and continuum of care needs
  • Participate in quality initiatives, outreach programs, and team collaboration
What You Bring:
  • Active New York State NP license and DEA certification
  • Board Certified (or eligible, with certification within 18 months)
  • Strong clinical judgment and commitment to evidence-based care
