What You'll Do:
As a key clinical leader, you will provide comprehensive primary care services, including diagnosis, treatment, prevention, and care coordination. You'll work closely with physicians and interdisciplinary team members to ensure high-quality, patient-centered care.- Perform patient assessments, histories, and physical exams
- Diagnose conditions and develop individualized treatment plans
- Prescribe medications in accordance with NYS guidelines
- Perform minor procedures and manage chronic conditions
- Review labs, imaging, and patient communications with timely follow-up
- Coordinate referrals and continuum of care needs
- Participate in quality initiatives, outreach programs, and team collaboration
What You Bring:
- Active New York State NP license and DEA certification
- Board Certified (or eligible, with certification within 18 months)
- Strong clinical judgment and commitment to evidence-based care
