We are a growing, patient-centered primary care practice with multiple locations in Hudson Valley including — Maybrook, NY and New Windsor, NY — seeking a dedicated and motivated Nurse Practitioner to join our team. What You'll Do Provide comprehensive primary care services to a diverse adult patient population Perform routine and preventive care, chronic disease management, acute illness evaluation, and health screenings Order and interpret diagnostic tests, prescribe medications, and develop individualized treatment plans Collaborate with the supervising physician and care team to deliver evidence-based, coordinated care Maintain accurate and timely documentation in the EMR Split time between our Maybrook and New Windsor office locations Schedule Full-Time or Part-Time positions available — we will work with the right candidate One required late evening per week (schedule flexibility surrounding this shift) Enjoy a predictable, balanced schedule designed to support work-life harmony No hospital rounding, no on-call requirements What We're Looking For Current NY State licensure as a Nurse Practitioner (NP) Minimum 1-2 years of experience in a primary care or family medicine setting Strong clinical assessment, diagnostic, and patient communication skills Comfortable working independently while collaborating within a team-based model Experience with electronic medical records (EMR) Bilingual candidates encouraged to apply
