We are seeking a compassionate Nurse Practitioner to join our growing team of dedicated professionals at the Albany Med Center of Excellence for Alzheimer's Disease! This is a great opportunity to join our multidisciplinary team to make a profound impact on advancing care for dementia patients in the region. The Nurse Practitioner will work in close collaboration with the MDs, nurses, social workers, and other team members to deliver compassionate and top quality patient centered care for our dementia patients. The Nurse Practitioner will work with physicians to evaluate and manage all clinical patient needs. The Nurse Practitioner will contribute to programmatic development, outreach, patient navigation, and quality initiatives of the department. The Albany Med Center of Excellence for Alzheimer's Disease provides advanced and subspecialized dementia care for patients throughout the region including the monoclonal antibody infusion treatments as well as comprehensive case management support.
Essential Duties and Responsibilities include:
Assesses, plans, implements and evaluates care for individual patients Assesses patient/family needs and provides education Promotes wellness and recommends community resources to meet patient and family needs Promotes/provides staff education Incorporates evidence-based research into clinical practice Knowledgeable of reimbursement procedures and documentation requirements With the support of the Center social workers, provides case management for patients and their families Serves as a resource person for the nursing staff May mentor new APPs Works in collaboration with other services (radiology, apheresis, laboratory) to coordinate care for dementia patients undergoing monoclonal antibody infusion treatments Qualifications Master's Degree of an accredited Nurse Practitioner program
  • required Able to quickly master all aspects of a computerized medical recordkeeping and billing system. Must project a professional image and earn the confidence of others. Excellent interpersonal, verbal, and written skills. Must be HIPAA compliant at all times. NP
