Job Description
Minimum Qualifications Education:
Master's degree in Nursing from an accredited Nurse Practitioner program. Current New York State license as a Family Nurse Practitioner.Experience:
Two years of experience providing nursing services preferred. Must be eligible for full and unconditional participation in Medicaid and Medicare programs.Certifications/Licenses:
Current DEA certification. ANCC certification preferred. Must maintain all required licensure, registration, and accreditation.Work Eligibility:

About the Role The Family Nurse Practitioner provides primary medical care, conducts assessments, develops care plans, and collaborates with physicians and multidisciplinary teams to support clients across shelter programs. Primary Responsibilities- Provide primary health care to assigned consumers.
- Perform history and physical examinations and preventive health measures.
- Order, interpret, and evaluate diagnostic tests.
- Develop and update care plans in collaboration with physicians.
- Prescribe or recommend medications and therapeutic treatments.
- Refer consumers to specialized health services as needed.
- Assist in mental status assessments and behavioral evaluations.
- Perform crisis intervention using verbal and nonverbal techniques.
- Report serious incidents immediately and follow required procedures.
- Assist in developing nursing policies and orienting new nurses.
- Attend clinical meetings, supervision, and professional development trainings.
- Perform additional job-related duties as assigned.
