• Are you a Primary Care Nurse Practitioner looking for an opportunity to make a meaningful difference outside of a traditional medical office?
• We are seeking an experienced Primary Care Nurse Practitioner to join an innovative Neighborhood Navigator Street Medicine Program serving individuals experiencing homelessness and substance use throughout Manhattan.
• Approximately 80% of this position is community-based. You will work alongside an established team of Neighborhood Navigators to provide medical care directly in the communities where clients live and spend their time.
• This is a unique, relationship-focused position. You will not be expected to work in a high-volume clinic or meet a traditional daily patient quota.
• Instead, you will have the time to build trust, address immediate medical needs, and help individuals connect with ongoing healthcare and supportive services.

What You'll Do:

• Provide primary and basic medical care in community and street-based settings.
• Perform medical assessments and monitor clients' physical health.
• Diagnose and manage common acute and chronic medical conditions.
• Provide basic wound care, vital signs, and other immediate medical services.
• Prescribe and manage medications within your scope of practice.
• Prescribe buprenorphine for individuals with opioid use disorder.
• Build trusting relationships with individuals experiencing homelessness and substance use.
• Connect clients with primary care, specialty care, behavioral health, substance use treatment, housing, safe havens, shelters, and other community resources.
• Accompany established clients to medical appointments when needed to help them successfully connect with ongoing care.
• Work collaboratively with Neighborhood Navigators and other members of the care team.
• Complete clinical documentation and administrative responsibilities.
• Provide care using a harm-reduction, trauma-informed, and person-centered approach.

The program serves several Manhattan neighborhoods, including:

• Washington Heights & Inwood
• Harlem
• Midtown / Garment District
• Chelsea

Lower East Side & Chinatown What We're Looking For:

• Current New York State Nurse Practitioner license
• National board certification in an appropriate NP specialty
• Primary Care, Family Nurse Practitioner (FNP), or Adult-Gerontology Primary Care NP experience
• Strong clinical assessment and decision-making skills
• Experience treating acute and chronic medical conditions
• Experience prescribing and managing medications
• Experience with buprenorphine/MOUD is strongly preferred
• Experience working with individuals experiencing homelessness, substance use, behavioral health needs, or other complex social needs is highly desirable
• Strong communication and relationship-building skills
• Ability to work comfortably in community and street-based environments
• Flexible and adaptable approach to patient care
• Ability to work independently while collaborating with an interdisciplinary team
• Bilingual English/Spanish skills are a plus.
