Immediate Hiring | Nurse Practitioner (NP) We are seeking an experienced and compassionate Nurse Practitioner (NP) to provide exceptional medical care to our nation's veterans in a skilled nursing and rehabilitation setting.

Oxford, NY 13830

Position:

Nurse Practitioner (NP)

Schedule:

Monday - Friday | 8:00 AM - 4:30 PM (Day Shift)

On-Call:

24/7 coverage required when no provider is onsite Key Responsibilities Perform routine and as-needed resident evaluations Assess, diagnose, and manage acute and chronic medical conditions Coordinate care with physicians, nursing staff, and interdisciplinary healthcare teams Develop and implement individualized treatment plans Maintain accurate documentation in the electronic medical record (EMR) system Participate in resident care planning and quality improvement initiatives Ensure continuity of care through onsite and on-call medical coverage Collaborate with facility leadership to maintain high standards of resident care and clinical outcomes Qualifications Active Nurse Practitioner (NP) License from the state of New York Strong clinical assessment, diagnostic, and treatment planning skills Experience in long-term care, skilled nursing, geriatrics, rehabilitation, or primary care preferred Excellent communication, leadership, and collaboration skills Commitment to providing compassionate, patient-centered care to veterans and their families Monday-Friday daytime schedule Opportunity to serve Veterans, Veteran Spouses, and Gold Star Parents Collaborative interdisciplinary healthcare environment Rewarding opportunity in a respected New York State Veterans Home #Hiring #NursePractitioner #NPJobs #LongTermCare #VeteransCare #SkilledNursing #HealthcareJobs #AdvancedPracticeProvider #NowHiring #NewYorkJobs #
