The Senior Community Care (SCC) program focuses on coordinating and delivering comprehensive clinical services to some of our highest-risk health plan members. Primary Responsibilities
Complete comprehensive patient assessments, including:
Medical history review
Medication reconciliation
Physical examinations and vital signs
Evidence-based screenings and point-of-care testing, as appropriate
Support members during short-term sub-acute rehabilitation stays
Maintain accurate and thorough clinical documentation, including ICD-10 coding
Collaborate closely with interdisciplinary care teams to coordinate patient care
Lead advanced illness and goals-of-care discussions
Educate and counsel patients and caregivers to support informed health care decisions
