Position Overview Primary Care Nurse Practitioner with 1+ years of nursing home or geriatric experience needed to provide primary care services at nursing home facilities in Tarrytown and Throggs Neck, NY. Compensation $56,000 annually Schedule Flexible scheduling! 12 hours per week Location Nursing homes in Tarrytown and Throggs Neck, NY. Key Responsibilities Monthly CCM follow‑ups — Provide structured, chronic care check‑ins for eligible residents. Care plan creation & updates — Develop and maintain comprehensive, patient‑centered care plans for chronic conditions. Medication reconciliation — Review medications monthly to ensure safety, accuracy, and adherence. Coordination with specialists — Communicate with cardiology, psychiatry, physiatry, and other specialists involved in chronic disease management. Monitoring chronic conditions — Track symptoms, labs, vitals, and disease progression for residents with two or more chronic illnesses. Documentation for CMS billing — Record time spent, care coordination activities, and care plan updates to meet CCM billing standards. Required Qualifications Active Nurse Practitioner license in N ew York with Board Certification. 1+ year of nursing home or long‑term care experience .
