Full Job Description Role Overview:
A Nurse Practitioner (NP) or Physician Assistant (PA) is sought to provide comprehensive family medicine services in a team-based primary care setting.
Key Responsibilities:
Conduct patient evaluations, manage acute and chronic conditions, order and interpret diagnostic tests, prescribe medications, coordinate referrals, and deliver preventive care and patient education. Collaborate with healthcare team members to ensure continuity of care and maintain accurate clinical documentation.
Practice Highlights:
Flexible scheduling supporting work-life balance, on-site behavioral health and laboratory services, modern facilities, and a diverse patient population.
Qualifications & Skills:
Active NYS license as an NP or PA, with relevant certifications. PAs require a physician assistant training program and certification; NPs need graduation from an approved nurse practitioner program and certification. Preferred candidates have ambulatory care experience, CPR certification, and strong collaborative skills.
Compensation:
Competitive salary starting at $125,000 plus benefits.
