Job Description

At Crossroads Hospice & Palliative Care, we believe that no one should face the end of life alone. Our mission is rooted in compassion, connection, and access-ensuring that patients and families receive the right care at the right time. As we reinvigorate our palliative care program, we're focused on reaching individuals earlier in their journey, offering support that can ease transitions, improve quality of life, and provide space for both healing and clarity. This role offers Nurse Practitioners the rare opportunity to build meaningful relationships during some of the most emotionally complex moments in a person's life. You'll walk with patients and families through uncertainty-holding space for grief, hope, planning, and honest conversations. With time to listen and the trust to guide, you'll help shape care that honors each person's experience, values, and needs.
Position Qualifications:
2 years of Nurse Practitioner experience in an acute care environment. Home Health, Palliative Care, or Hospice Experience Required. Masters of Science in Nursing. Active unencumbered OH NP license. Current driver's license and auto liability coverage. Detailed assessment skills as part of this role require recertification for Hospice patients. Interest in a community-based role that requires travel between patient visits. Drug Enforcement Agency (DEA) number Required. National Provider Identifier (NPI) Required.

Shift Availability:
