October 5, 2025 Position Summary The Nurse Practitioner provides professional, comprehensive primary medical care to patients of Compassion Health Toledo in collaboration with staff physicians and the care team. This position focuses on patient-centered, high-quality healthcare and works closely with multidisciplinary team members to promote continuity of care, clinical excellence, and positive health outcomes. Essential Duties & Responsibilities Obtain health histories and perform physical exams to develop accurate diagnoses and treatment plans. Order and interpret diagnostic tests, including laboratory work and imaging. Prescribe medications and treatments according to clinical practice guidelines and patient needs. Collaborate with physicians, nurses, care coordinators, and other healthcare professionals to ensure continuity of care. Provide counseling and education to patients and families regarding physical and developmental well-being, treatment options, and preventive care. Identify and connect patients to community health and social service resources as appropriate. Participate in staff meetings, provider meetings, and interdisciplinary case reviews. Document all clinical encounters promptly and accurately in the electronic medical record (EMR). Maintain compliance with all federal and state regulations, including HIPAA, OSHA, and CLIA. Participate in peer review, continuing education, and performance improvement initiatives. Maintain communication with administration regarding licensure, certifications, or any disciplinary matters. Meet semi-annually with the collaborating physician as required by state supervision protocols. Perform other duties as assigned to support clinical excellence and the mission of the organization. Qualifications Graduate of an accredited Nurse Practitioner program; Master's Degree preferred. Current Ohio RN license and Nurse Practitioner certification or registry eligibility required. Eligible for Certificate of Fitness (prescriptive privileges) in Ohio. Demonstrated ability to work independently and make clinical assessments. Experience providing care for adult, pediatric, or family patient populations. Excellent communication, critical thinking, and decision-making skills. Commitment to delivering high-quality, compassionate, and patient-centered care. Skills & Competencies Strong clinical judgment and attention to detail. Ability to collaborate effectively within a multidisciplinary team. Proficiency in using electronic medical records (EHR). Professional integrity, reliability, and accountability. Commitment to ongoing learning and quality improvement. Ability to maintain confidentiality and adhere to ethical standards. Physical Demands & Work Environment Regularly required to sit, stand, walk, and communicate effectively. Occasional lifting up to 25 pounds.
