This practice was one of the first to transition to the patient centered medical home model, putting patients at the forefront of the practice's decisions and leveraging technology like the patient portal, digital intake, and scheduling to streamline processes and communication. Non-Clinical Work, Handled:
Focus on practicing medicine while CCP's local centralized management services organization takes care of administrative, billing, and operational functions—so you can spend more time doing what you do best. \n Work life balance with predictable hours, no evenings or weekends for APPs, disconnect on vacations and holidays, workflows that support efficiency. \n \n \n Responsibilities \n \n Practice medicine with physician supervision including conducting examinations and writing prescriptions. \n Collaborate with physicians in managing acute and long-term medical needs of patients. \n Assess patient status by obtaining health history. \n Order/perform laboratory diagnostic and other screening tests. \n Treat illnesses, give injections, suture wounds. \n Monitor therapies and provide continuity of care. \n Counsel patient/family on preventive health care. \n Participate in peer reviews, chart reviews, staff education, clinical guideline development and other continuing education. \n \n \n If you're passionate about delivering high-quality patient care in a dynamic, multi-specialty setting, please

Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills.
