PRN NURSE PRACTITIONER A
Path of Care Hospice in Weatherford, OK is seeking a PRN Nurse Practitioner to support the medical component of our hospice program and provide compassionate, patient‑centered care. What You'll Do as a Nurse Practitioner As a PRN Nurse Practitioner, you play a vital role in ensuring high‑quality medical oversight and continuity of care.Responsibilities include:
Conducting patient visits, physical examinations, and required Face‑to‑Face encounters Completing timely and accurate documentation Collaborating with the Administrator/Director, Medical Director, and other healthcare professionals to develop and refine patient care policies Working closely with the Patient Care Coordinator to ensure care meets professional standards and regulatory requirements Providing guidance to the Interdisciplinary Team (IDT), Medical Director, and Ethics Committee when complex or ethical issues arise Assisting with federal, state, or local surveys and inspections as needed Helping develop in‑service education and training materials for clinical staff Your work ensures patients receive compassionate, coordinated care while supporting the clinical team with expertise and leadership. Qualifications Nurse Practitioner legally authorized to practice in Oklahoma with a license free of sanctions Experience in geriatrics, palliative medicine, or a related field Hospice experience preferred Ability to work in the field 95% of the time, including frequent bending, stooping, sitting, standing, and walking Work Schedule PRN Flexible hours, Monday through Friday Ready to Join Our Healthcare Team?
