Required Qualifications NP:
MSN or DNP degree with AANP or ANCC certificationPA:
Graduate of an accredited Physician Assistant program with NCCPA certification Active, unrestricted state license (or ability to obtain) DEA license/prescriptive authority (or ability to obtain) Ability to obtain a collaborative practice agreement if required Valid driver's license and reliable transportation Ability to perform physical assessments and travel between patient care locations Preferred Qualifications 1+ years of clinical experience, preferably in long-term care, geriatrics, or senior living Experience caring for medically complex patients Knowledge of geriatrics, chronic illness, and end-of-life care discussions EMR proficiency and strong documentation skills Highlights Facility-based senior care role focused on high-risk patient populations Value-based care emphasizing quality outcomes rather than visit volume Strong interdisciplinary team support No call, weekends, or holidays Opportunity to build meaningful relationships with seniors and make a significant impact on patient outcomes through coordinated, longitudinal care. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
