Job Description
We are seeking a qualified Nurse Practitioner (Skilled Nursing) to join our team in a Skilled Nursing Facility setting. Serve as a clinical leader, collaborating with the care team to oversee short- and long-term residents. Handle admissions, discharges, stability checks, and evaluations for condition changes. Perform advanced care planning, comprehensive assessments, and prescribe treatments.
