Full-time / Field-Based About the Role Are you a compassionate Nurse Practitioner who values patient relationships over office quotas? We are a dedicated house-calls practice providing comprehensive primary care to patients where they are most comfortable—in their private homes and assisted living facilities. We are looking for a self-motivated NP to join our team. You will serve as the primary provider for patients in Northern Palm Beach County and Hobe Sound , ensuring that those with mobility or transportation challenges still receive world-class medical attention. A Day in the Life As a mobile provider, your "office" changes daily. You will manage a local route, visiting patients in private homes and senior living communities. You'll have the autonomy to manage your clinical day while providing the kind of deep, attentive care that isn't possible in a high-volume clinic setting.

Key Responsibilities Comprehensive Care:

Perform physical exams, diagnose and manage chronic/acute conditions, and prescribe medications.

Mobile Visits:

Travel to residences and facilities to provide high-quality primary care.

Patient Advocacy:

Coordinate with families, caregivers, and facility staff to ensure continuity of care.

Clinical Autonomy:

Manage your daily schedule and patient load independently.

Documentation:

Maintain accurate, timely electronic health records (EHR).

Compensation & Benefits Competitive Salary:

$100,000 - $125,000 (commensurate with experience).

Board Certification ( FNP, AGNP, or ANP ). Valid Driver's License and reliable transportation. Strong clinical judgment and the ability to work independently. Excellent communication skills and a passion for the geriatric/homebound population.
