Job Overview Join our dynamic healthcare team as a Mobile Nurse Practitioner, where you will deliver comprehensive, patient-centered care directly in the community. This role offers the opportunity to work across diverse settings such as assisted living facilities, group homes, and home care environments. You will utilize your advanced clinical skills to assess, diagnose, and manage a wide range of health conditions, ensuring seamless coordination with healthcare professionals to optimize patient outcomes. Your energetic approach and dedication will make a meaningful difference in the lives of patients who need flexible, accessible medical services. Responsibilities Conduct thorough physical examinations and patient assessments in various community settings, including home visits and assisted living facilities. Manage acute and chronic conditions with expertise in primary care, geriatrics, and behavioral health. Administer injections, including immunizations and perform suture removal. Utilize electronic health record (EHR) systems to document medical records accurately and efficiently. Collaborate closely with healthcare professionals such as physicians, specialists in gastroenterology, infectious disease care providers, and case managers to develop comprehensive treatment plans. Provide health coaching, medication management, and education tailored to individual patient needs across diverse populations including seniors, adolescents, and individuals with disabilities. Perform diagnostic evaluations using laboratory testing, sonography, or medical imaging techniques; collect specimens for processing; and interpret results for timely decision-making. Support telehealth initiatives by delivering virtual consultations that maintain high-quality patient care outside traditional clinical settings. Qualifications Valid Nurse Practitioner license with current certification to practice independently or under collaborative practice agreements. Proven experience working in diverse clinical environments such as hospital medicine, emergency rooms, ICU/critical care units, or outpatient clinics. Strong proficiency in EMR/EHR systems; familiarity with CPT coding, ICD-10/ICD-9 coding standards is highly desirable. Excellent client communication skills with the ability to educate patients effectively across age groups—pediatrics through geriatrics—and manage complex cases involving multiple comorbidities. Ability to work independently while collaborating seamlessly with multidisciplinary teams to deliver exceptional patient care.
