Job Summary:
Carease Health - PA is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team of dedicated healthcare professionals. As a Nurse Practitioner, you will be responsible for delivering high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home. Our hybrid model is designed to provide flexibility and work-life balance while supporting your professional growth and development.
Key Responsibilities:
Provide comprehensive preventative care to residents in skilled nursing facilities, focusing on proactive measures for enhanced physical health and well-being Conduct on-site visits and remote monitoring using software and technology to ensure seamless patient care Collaborate with healthcare providers and facility staff to develop and implement individualized care plans Participate in the Chronic Care Management (CCM) and Collaborative Care Management (CoCM) programs to address mental health and well-being Stay up-to-date with industry best practices and maintain accurate and timely documentation Build strong relationships with facility staff, patients, and families to ensure patient-centered care
Requirements:
Active NP or PA license in the state of practice 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills
