Nurse Practitioner (NP) or Physician Assistant (PA) in-Person/ Telemed Onyx Medical Group Inc Torrance, CA Job Details Part-time | Full-time | Contract $8,000 a month 1 hour ago Benefits Mileage reimbursement Flexible schedule Qualifications BLS Certification Care documentation Driver's License End-of-life care DEA License Advanced practice provider Urgent care Time management Full Job Description Onyx Medical Group, Inc. is seeking a compassionate, dependable, and clinically strong Nurse Practitioner (NP) or Physician Assistant (PA) to provide mobile, in-person medical and Telemed care to patients in the comfort of their homes, assisted living facilities, board and care homes, and other residential settings. This position is ideal for a provider who is passionate about caring for homebound, elderly, and medically complex patients and who is comfortable working independently in the field while collaborating closely with the physician team and office staff. Our group focuses on improving quality of life through comprehensive mobile care, including primary care services, annual wellness visits, medication management, referrals, and coordination of home-based diagnostic and specialty services . Key Responsibilities Travel to patient homes, residential care facilities, and assisted living communities to provide in-person medical visits Perform comprehensive patient assessments, history, and physical exams Provide primary care services and follow-up care for adult and geriatric patients Conduct annual wellness visits and routine medical evaluations Assess acute and chronic medical conditions and develop treatment plans Manage medication reconciliation, prescription refills, and medication adjustments as appropriate Order and coordinate laboratory work, mobile X-rays, ultrasounds, echocardiograms, and other diagnostic services Evaluate and initiate referrals for home health, specialty care, diagnostic services, wound care, podiatry, and DME/medical supplies Coordinate care with physicians, caregivers, family members, home health agencies, specialists, pharmacies, and facility staff Recognize changes in patient condition and escalate cases appropriately when physician involvement is needed Complete accurate and timely EMR documentation for every patient encounter Maintain compliance with all clinical protocols, company policies, payer requirements, and state regulations Deliver compassionate, respectful, and patient-centered care in every setting Qualifications Active and unrestricted California Nurse Practitioner or Physician Assistant license Board certification required DEA license and furnishing number/prescriptive authority, as applicable Current BLS certification Valid driver's license, reliable transportation, and active auto insurance Ability and willingness to travel throughout assigned service areas Strong clinical judgment and assessment skills Excellent communication, bedside manner, and documentation skills Comfortable working independently in the field Professional, organized, and dependable Preferred Qualifications Minimum 1-2 years of experience as an NP or PA preferred Experience in primary care, family medicine, geriatrics, home-based care, internal medicine, palliative care, wound care, urgent care, or post-acute care Experience working with homebound, elderly, or medically complex patients Bilingual candidates are encouraged to apply Prior mobile medical or home visit experience is highly preferred Ideal Candidate We are looking for someone who: Is compassionate and truly enjoys caring for seniors and homebound patients Can work independently and efficiently in a mobile setting Has strong time management and route-planning skills Is confident in handling complex patients in non-clinic environments Can build trust with patients, caregivers, and families Understands the importance of timely charting, follow-up, and care coordination Wants to grow with a professional and expanding mobile physician group What We Offer Competitive compensation Mileage reimbursement Flexible scheduling options Supportive clinical and administrative team Opportunity to work in a meaningful, patient-centered model of care Growth opportunities within a growing mobile medical group About Onyx Medical Group, Inc.
