Our team of board-certified physicians, nurse practitioners, and physician assistants works across hospitals, rehabilitation centers, assisted living communities, and even patients' homes to ensure seamless transitions and compassionate outcomes. Job Summary We are seeking a dynamic and compassionate Nurse Practitioner or Physician Assistant to join our dedicated healthcare team within an assisted living setting. Schedule:
Monday through Friday, with rotating weekend call coverage taken from home. Daily visit volume and community assignments are set by Clinical Operations.Travel:

An assisted living community is a licensed residential setting where older adults live in their own apartments and receive help with activities of daily living, medication assistance, meals, and supervision. Unlike a skilled nursing facility, an assisted living community is not staffed with twenty-four-hour licensed nursing coverage and is not built around short-term rehabilitation. Residents are medically complex and frail, but they are at home, and the clinical goal is to keep them there. Care is delivered at the resident's bedside or in the resident's apartment, and the provider functions as the resident's primary care provider rather than as a consulting or rounding specialist. Success in this setting is measured by early detection of change in condition, treating in place whenever it is clinically appropriate, aligning care with the resident's stated goals, and preventing avoidable emergency department visits, hospitalizations, and readmissions. Clinical Assessment and Medical Management Serve as the primary care provider for an assigned panel of assisted living residents, providing transition, palliative, and long-term in-residence visit services for Geriatric Post-Acute Specialists. Conduct in-residence assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam. Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment. Identify diagnoses to be used in care management and active medical management in the furtherance of treatment. Order, perform, and interpret diagnostic studies within scope of practice, including point-of-care testing, mobile laboratory, and mobile imaging services. Prescribe, adjust, and monitor medications in accordance with the provider's Texas prescriptive authority agreement and applicable federal and state law. Manage chronic conditions prevalent in the geriatric population, including heart failure, COPD, diabetes, hypertension, chronic kidney disease, atrial fibrillation, dementia and related behavioral expressions, depression and anxiety, osteoarthritis, and chronic pain. Assess and manage the geriatric syndromes that drive decline in assisted living, including falls and fall risk, functional and cognitive decline, unintended weight loss and malnutrition, incontinence, skin integrity and wound care, delirium, and polypharmacy. Perform structured medication reviews and lead appropriate deprescribing consistent with recognized geriatric prescribing guidance (for example, the AGS Beers Criteria) and the resident's stated goals of care. Identify urgent and emergent situations and intervene appropriately, including timely escalation, physician consultation, and transfer when clinically indicated. Evaluate and treat acute changes in condition on site whenever clinically appropriate and consistent with the resident's goals, in order to reduce avoidable emergency department visits, hospitalizations, and readmissions. Recognize when a resident's needs exceed the licensed service capability of the assisted living community and coordinate transition to the appropriate level of care. Transitional, Palliative, and Advance Care Planning Complete post-hospital and post-skilled-nursing transition visits within established timeframes; reconcile discharge medications, confirm follow-up appointments, and close identified gaps in care. Conduct advance care planning conversations with residents and their legally authorized representatives; complete and document advance directives, Out-of-Hospital Do-Not-Resuscitate orders, and medical orders for treatment consistent with Texas law and Employer policy. Deliver palliative services focused on symptom management, goals-of-care alignment, and family and caregiver support. Identify residents appropriate for hospice referral, initiate the conversation, and coordinate the transition with the hospice provider and the community. Communicate findings from your assessment that will be used to inform the resident's PCP, treating specialists, and health plan of potential gaps in care. Care Coordination and Community Partnership Serve as the clinical point of contact for the community's executive director, wellness or health services director, and medication aides; provide orders, clarification, and education that support safe care within the community's licensed scope of service. Participate actively in interdisciplinary team (IDT) meetings, resident care conferences, and change-of-condition huddles, and collaborate closely with community administration. Communicate proactively and compassionately with residents, families, and legally authorized representatives regarding change in condition, treatment plans, and goals of care. Educate residents, families, and community staff on topics such as disease process, medication and compliance, infection prevention, fall reduction, and when to escalate a change in condition. Coordinate services with home health, hospice, therapy, pharmacy, durable medical equipment, laboratory, and mobile imaging vendors. Remote Patient Monitoring and Chronic Care Management Geriatric Post-Acute Specialists offers remote patient monitoring (RPM) and chronic care management (CCM) services in partnership with a contracted partner organization. These programs are active in a number of the communities we serve today and are expected to expand to additional communities over time. Participate in the management of residents enrolled in the RPM and CCM programs, including identifying and enrolling appropriate residents, reviewing transmitted data and partner-generated alerts, and adjusting the plan of care as clinically indicated. Respond to clinical escalations from the partner organization within established timeframes and document the clinical response in the EHR. Coordinate with the partner's care management staff so that monitoring parameters, outreach, and care plans remain aligned with the resident's treating plan and stated goals of care. Complete the documentation required to support RPM and CCM services and associated billing requirements. Documentation, Coding, and Compliance Follow Medicare and state guidelines and Employer policy and procedures when providing services and submitting charges into the EHR for billing of services. Complete all required documentation for billing and documentation of services rendered by established deadlines. Select the accurate place of service and evaluation and management level for the assisted living setting Document to the level of specificity required for accurate risk adjustment and annual reassessment of chronic conditions. Maintain compliance with HIPAA, Medicare requirements, federal fraud and abuse law, Texas Board of Nursing rules, and applicable Texas Health and Human Services assisted living regulations.Required Qualifications:
