Carease Health - PA is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a valued member of our team, you will be responsible for providing patient-centered care, building strong relationships with facilities, and collaborating with our dedicated scribe team for efficient documentation.
Responsibilities:
  • Provide high-quality preventative care to residents in skilled nursing facilities
  • Conduct on-site visits and remote work from home, supported by a dedicated scribe team
  • Build strong relationships with facilities and maintain a patient-centered approach
  • Collaborate with our clinical directors, regional managers, and supervisors for support and training
  • Conduct documentation review and ensure accurate and timely recording of patient data
Requirements:
  • Active NP or PA license in the state of practice
  • 1+ year of experience in Geriatric care and documentation review
  • Excellent communication, collaboration, and teamwork skills

PART_TIME only 15 hours per week

$41000 - $43000
