Carease Health - PA is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a valued member of our team, you will be responsible for providing patient-centered care, building strong relationships with facilities, and collaborating with our dedicated scribe team for efficient documentation.Responsibilities:
- Provide high-quality preventative care to residents in skilled nursing facilities
- Conduct on-site visits and remote work from home, supported by a dedicated scribe team
- Build strong relationships with facilities and maintain a patient-centered approach
- Collaborate with our clinical directors, regional managers, and supervisors for support and training
- Conduct documentation review and ensure accurate and timely recording of patient data
Requirements:
- Active NP or PA license in the state of practice
- 1+ year of experience in Geriatric care and documentation review
- Excellent communication, collaboration, and teamwork skills
