Job Description
Overview Marin Community Clinics, founded in 1972, is today, a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty and referral services. As a Federally Qualified Health Center (FQHC), we provide vital health services to almost 40,000 individuals annually in Marin County. The Clinics regularly receive national awards from the Health Resources and Services Administrations (HRSA). Our Mission is to promote health and wellness through excellent, compassionate care for all. Reporting to a supervising Medical Director, the Hybrid Primary Care Practice Partner functions as an extension of the on-site, in-clinic primary care team, primarily supporting the patient panel of 1-3 designated providers in a specific primary care site or Pod. This provider will provide comprehensive, high-quality primary care services as clinically appropriate and feasible through a variable mix of in-person and virtual care options including video and telephonic health visits, as well as through asynchronous communication modalities available in the electronic health record. Responsibilities Clinical Responsibilities In collaboration with in-clinic primary care providers (PCPs), provide high quality, evidence based comprehensive virtual primary care Conduct a thorough history and assessment of symptoms virtually and develop treatments and follow up plans consistent with evidence-based medicine. Perform urgent care and medical triage visits (in-person or virtual) as needed in response to triage by call center representatives and the triage nurse team. Support the Transitions of Care nurse team by addressing urgent patient care and care coordination needs for patients transitioning from acute care settings to home. Offer patient education, counseling, and follow-up care to promote wellness and chronic disease management Assist PCPs in electronic medical record (EHR) inbox management through outreach to patients with abnormal lab, radiology, or other results and develop a clinically appropriate management and follow up plan for patients Complete documentation in the electronic health record (EHR) within MCC timeliness standards. Update patient problem lists, manage referrals, and perform other non-direct patient asynchronous clinical care as appropriate Provide medical care in a trauma-informed, and culturally sensitive way. Qualifications Education and Experience Advanced Practice Provider (NP/PA) from an accredited medical training program; current California medical license and DEA certificate required. Minimum of 3 years of experience providing primary care in a family or community health setting is required. Experience with EHR systems (EPIC preferred). Prior experience working in a Federally Qualified Health Center (FQHC) a plus.

Demonstrated ability to be proactive, flexible, and multitask in a busy clinical environment Willingness to receive regular feedback and solve problems in real-time Bilingual (Spanish/English) desired. Excellent written and verbal communication skills Commitment to diversity, equity, inclusion, and trauma-informed care principles. Active BLS Certification. Must maintain Continued Medical Education (CME) as required for certification renewals.Physical Requirements and Working Conditions:
Fullfill immunization and fit for duty regulatory requirements. Must be able to navigate electronic medical records and other related IT systems efficiently with training Must have ability to perform confidential patient visits remotely including reliable, high-speed internet/telephonic services and a remote workspace that protects patient confidentiality Expect prolonged standing and/or prolonged periods sitting at desk and working on a computer. Must be available for on-site training as needed during onboarding or as part of on-going medical credentialing with the acknowledgement that supervising Medical Director will work to accommodate reasonable scheduling requests.
