Milford, Connecticut/hybrid Telehealth •Competitive 1099 compensation structure based on experience and patient volume Elevate Therapy is seeking a compassionate and collaborative Psychiatric Mental Health Nurse Practitioner (PMHNP/APRN) to join our growing outpatient mental health practice serving Connecticut and New York clients through in-person and Telehealth services. We are looking for a provider who values thoughtful, relationship-based psychiatric care and enjoys collaborating with therapists to support clients through a comprehensive, whole-person approach. We are looking for a Provider who: Has completed independent licensure requirements and does not require physician collaboration Is licensed as a Psychiatric Mental Health Nurse Practitioner (PMHNP) in Connecticut Is comfortable working in a private practice setting as an independent contractor (1099), or interested in transitioning into private practice Has experience conducting psychiatric evaluations, providing medication management, and treatment planningValues a comprehensive approach that considers biological, psychological, and environmental factors affecting mental health Is knowledgeable in conductingADHD evaluations and ordering appropriate labs when clinically indicated Is interested in working with children, adolescents, and/or adults Collaborates effectively with therapists and other healthcare providers to provide coordinated care Demonstrates compassion, professionalism, strong communication skills, and clinical excellence Role responsibilities: Conduct psychiatric evaluations and follow-up medication management appointments Develop individualized treatment plans and document patient progress Prescribe and manage medications as medically necessary Order and review labs as medically necessary Collaborate with other care providers to support integrated patient care Maintain timely and accurate clinical documentation Ready to Join Us?
