Role Overview:
Nurse Practitioner, Neurology and Memory Care We are seeking a skilled and compassionate Nurse Practitioner who is licensed in Connecticut to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment. Location Remote Job Description Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies Partner with our neurologist(s) and broader care team to ensure continuity of care Educate patients and families on diagnoses, treatment plans, and disease progression Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements Key Accountabilities Active, unrestricted NP license in Connecticut ; certification in geriatrics, adult-gerontology, or family practice preferred 2+ years of clinical experience in neurology, memory care, and/or dementia care settings Strong clinical judgement and clinical documentation skills Comfortable managing a full patient panel independently with physician collaboration Excellent communication skills with patients facing cognitive decline and their caregivers The Local Infusion Way Local Infusion is a respectful, upbeat, and remote-first team united by our mission of shaping the way specialty infusion care is delivered.
