Job Summary We are seeking a dedicated and highly skilled Wound Care Nurse Practitioner to join our dynamic healthcare team. In this vital role, you will provide expert wound management and advanced nursing care to diverse patient populations, including those in acute, post-acute, and long-term care settings. Your expertise will help improve healing outcomes, reduce complications, and enhance patient quality of life through comprehensive assessment, innovative treatment plans, and collaborative practice. Responsibilities Conduct thorough patient assessments, including wound evaluation, vital signs, and medical history review using electronic health record (EHR) systems such as Epic or eClinicalWorks. Develop and implement individualized wound care treatment plans incorporating advanced techniques like suturing, debridement, and dermal fillers when appropriate. Administer specialized therapies such as Botulinum toxin treatment for related conditions and manage complex cases involving infectious disease care or acute pain management. Collaborate closely with physicians, nurses, physical therapists, and other healthcare professionals to coordinate comprehensive patient care across settings including hospitals, nursing homes, assisted living facilities, and outpatient clinics. Perform procedures such as IV infusion, catheterization, tube feeding management, and sterile dressing changes while maintaining strict infection control standards. Document all patient interactions accurately in EMR/EHR systems like Cerner or Athenahealth, ensuring compliance with HIPAA regulations and CPT coding requirements. Educate patients and families on wound healing processes, self-care techniques, medication administration, and health coaching strategies to promote optimal recovery outcomes. Qualifications Current Nurse Practitioner license with certification in wound care or related specialties preferred; valid DEA registration for prescribing medications. Proven experience working as a nurse practitioner in diverse clinical environments such as hospital medicine, critical care (ICU/Level I & II trauma centers), or post-acute care settings. Demonstrated expertise in advanced procedures including suturing, phlebotomy, venipuncture, sterile processing techniques, and injection administration.
