In-person, telehealth, school-based, community-based, and other approved clinical settings Position Overview Rosalino Counseling Services, LLC is seeking a dedicated, compassionate, and experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing multidisciplinary behavioral health team. The PMHNP will provide comprehensive psychiatric evaluations, diagnostic assessments, medication management, patient education, and ongoing psychiatric care to diverse populations. Depending on the provider's training and experience, the position may include working with children, adolescents, adults, and older adults. The selected provider will collaborate closely with psychologists, clinical social workers, mental health counselors, therapists, primary care providers, schools, hospitals, attorneys, caregivers, and community organizations. The PMHNP may also provide individual psychotherapy when qualified, trained, and interested. This is an excellent opportunity for a psychiatric nurse practitioner who wants to develop a flexible and meaningful role within a supportive, culturally responsive, and growing mental health practice. Primary ResponsibilitiesPsychiatric Evaluations and Diagnosis Conduct comprehensive initial psychiatric evaluations and diagnostic assessments. Gather detailed psychiatric, medical, developmental, social, educational, family, trauma, substance-use, and medication histories. Evaluate symptoms, functioning, strengths, treatment needs, risk factors, and protective factors. Diagnose psychiatric and behavioral health conditions using current diagnostic criteria and accepted clinical standards. Assess and treat conditions that may include: Anxiety disorders Depressive disorders Post-traumatic stress disorder Attention-deficit/hyperactivity disorder Bipolar and related disorders Obsessive-compulsive and related disorders Psychotic disorders Sleep-related concerns Trauma-related conditions Behavioral and emotional disorders Autism-related psychiatric concerns Substance-use disorders Adjustment disorders Grief and loss Perinatal and postpartum mental health conditions Cognitive and age-related psychiatric concerns Other mental health and neurodevelopmental conditions within the provider's scope of practice Order, review, and interpret appropriate laboratory work, toxicology screening, medication levels, and other diagnostic studies within the provider's authorized scope. Evaluate whether physical health conditions, medications, substance use, or environmental factors may be contributing to psychiatric symptoms. Refer clients for psychological testing, neuropsychological evaluation, medical evaluation, specialty care, or a higher level of psychiatric treatment when clinically indicated. Medication Management Prescribe, manage, monitor, and discontinue psychiatric medications in accordance with applicable laws, professional standards, and the provider's scope of practice. Explain medication options, expected benefits, possible side effects, risks, alternatives, and monitoring requirements. Obtain informed consent before initiating or significantly changing medication treatment. Monitor treatment response, medication adherence, side effects, drug interactions, and overall functioning. Complete medication reconciliation and maintain an accurate record of current and previous medications. Coordinate with pharmacies, primary care providers, pediatricians, specialists, hospitals, laboratories, and caregivers as clinically appropriate. Use evidence-based prescribing practices and exercise particular care when prescribing controlled substances. Review applicable prescription-monitoring information when required or clinically appropriate. Educate clients and families about medication safety, storage, adherence, and potential warning signs. Develop safe medication plans for children, older adults, pregnant or postpartum clients, and individuals with complex medical conditions within the provider's training and competence. Document medication decisions, informed-consent discussions, clinical rationale, and monitoring plans thoroughly and promptly. Individual Behavioral Health Services Maintain an individual caseload for psychiatric care and medication-management appointments. Provide supportive therapy, psychoeducation, brief psychotherapy, or ongoing individual therapy when qualified and appropriate. Develop individualized, person-centered treatment plans based on each client's diagnosis, strengths, needs, preferences, cultural background, and treatment goals. Use evidence-based approaches such as motivational interviewing, trauma-informed care, solution-focused interventions, cognitive behavioral strategies, and supportive psychotherapy. Help clients develop coping skills, improve treatment adherence, understand their diagnoses, and participate actively in their recovery. Involve parents, guardians, caregivers, or family members when clinically appropriate and properly authorized. Monitor progress and revise treatment plans based on the client's response to treatment. Crisis Assessment and Risk Management Conduct suicide, self-harm, violence, substance-use, abuse, neglect, and safety-risk assessments. Develop appropriate safety plans with clients, parents, guardians, and caregivers. Provide crisis evaluation, stabilization, and clinical recommendations within the provider's scope. Determine when clients require emergency services, mobile crisis intervention, hospitalization, partial hospitalization, intensive outpatient care, detoxification, or another higher level of care. Coordinate with emergency departments, crisis teams, hospitals, law enforcement, child-protection agencies, and other authorized professionals when necessary. Follow all applicable mandated-reporting laws and organizational safety procedures. Document risk assessments, interventions, consultations, referrals, and follow-up plans clearly and promptly. Participate in post-hospitalization follow-up and continuity-of-care planning. Collaborative and Integrated Care Collaborate with therapists, psychologists, social workers, counselors, physicians, nurses, case managers, educators, and community providers. Coordinate medication management with clients' ongoing psychotherapy and behavioral health treatment. Communicate with primary care providers and medical specialists regarding health conditions that may affect psychiatric treatment. Participate in clinical consultation, case conferences, treatment-planning meetings, and interdisciplinary team meetings. Coordinate care with schools, residential programs, nursing facilities, hospitals, community agencies, and child-welfare professionals when appropriate authorization has been obtained. Provide clinically appropriate recommendations to therapists and other members of the treatment team. Support smooth transitions of care following hospitalization, crisis stabilization, or changes in the client's treatment needs. Make timely referrals for medical, psychological, neuropsychological, educational, substance-use, and community-based services. Child and Adolescent Psychiatry Applicants with pediatric psychiatric experience may: Evaluate and treat children and adolescents with emotional, behavioral, developmental, and psychiatric concerns. Collaborate closely with parents, guardians, pediatricians, therapists, educators, and school personnel. Review developmental history, family history, school functioning, behavioral concerns, previous testing, and current services. Consider the child's home, school, social, cultural, and developmental environment when developing treatment recommendations. Provide developmentally appropriate education regarding diagnoses and medications. Monitor growth, vital signs, sleep, appetite, behavior, academic functioning, and medication-related effects. Review IEPs, 504 Plans, psychological evaluations, neuropsychological reports, and other relevant records when authorized. Refer children for psychological, developmental, autism, ADHD, or neuropsychological testing when additional diagnostic clarification is needed. Obtain appropriate consent from parents or legal guardians and assent from minors when applicable. Adult and Geriatric Psychiatry Applicants with adult or geriatric experience may: Assess and treat adults and older adults experiencing psychiatric, cognitive, medical, social, or functional challenges. Address medication interactions, polypharmacy, chronic medical conditions, and age-related changes. Collaborate with primary care providers, specialists, caregivers, assisted-living communities, nursing facilities, and home-based service providers. Evaluate changes in memory, mood, behavior, sleep, functioning, and decision-making capacity within the provider's scope. Refer clients for neuropsychological testing, neurological evaluation, or other specialty services when clinically indicated. Support caregivers and families through education, consultation, and coordinated treatment planning. Provide services in outpatient, telehealth, residential, or community-based settings as permitted. Immigration and Trauma-Informed Services The PMHNP may have opportunities to work with immigrants, refugees, survivors of domestic violence, crime victims, trafficking survivors, and families experiencing immigration-related stress.

Responsibilities may include:

Providing trauma-informed psychiatric evaluations and treatment. Assessing the psychiatric effects of trauma, persecution, abuse, family separation, displacement, and fear of deportation. Managing psychiatric medications for clients experiencing PTSD, depression, anxiety, sleep disturbance, and other trauma-related symptoms. Providing psychiatric records, treatment summaries, or clinical documentation when appropriately authorized. Collaborating with the practice's clinicians who conduct immigration psychological evaluations. Providing clinically appropriate treatment recommendations and continuity of care following an immigration evaluation. Communicating professionally with authorized attorneys while maintaining client confidentiality and appropriate clinical boundaries. Participating only in immigration-related assessments, reports, or legal matters that fall within the PMHNP's education, competence, licensure, and authorized scope of practice. Documentation and Compliance Complete psychiatric evaluations, progress notes, treatment plans, medication records, refill documentation, risk assessments, and care-coordination notes accurately and on time. Maintain complete and confidential health records in accordance with HIPAA requirements. Follow applicable federal and state laws, licensing regulations, prescribing requirements, ethical standards, and organizational policies. Obtain and document informed consent for medication treatment and telehealth services. Protect client privacy during in-person, telehealth, school-based, and community-based services. Participate in quality-assurance, chart-review, and clinical-compliance activities. Complete prior authorizations, medication-related forms, treatment summaries, and other clinically necessary documentation. Maintain appropriate professional boundaries with clients, families, referral sources, and collaborating professionals. Remain current with continuing education, prescribing guidelines, evidence-based psychiatric practices, and professional requirements. Additional Responsibilities Participate in clinical consultation, supervision, team meetings, and case conferences. Assist with developing psychiatric policies, workflows, medication-management procedures, and safety protocols. Support the continued expansion of psychiatric services within the practice. Provide education and consultation to clinical team members regarding psychiatric diagnoses and medications. Participate in professional development and continuing education. Provide mentorship or supervision to qualified trainees or junior professionals when legally permitted and mutually agreed upon. Communicate professionally with clients, families, pharmacies, insurance companies, healthcare providers, and referral partners. Required Qualifications Master's or doctoral degree in nursing from an accredited psychiatric mental health nurse practitioner program. Current national board certification as a Psychiatric Mental Health Nurse Practitioner. Active Rhode Island Advanced Practice Registered Nurse license or eligibility to obtain Rhode Island licensure. Current authorization to prescribe medications in accordance with applicable Rhode Island requirements. Active DEA registration or eligibility to obtain one if the position includes prescribing controlled substances. Current controlled-substance registration or other prescribing credentials required by applicable law. Current professional liability insurance or willingness to obtain coverage before beginning clinical services. Current Basic Life Support certification if required for the assigned setting. Experience conducting psychiatric evaluations and providing medication management. Strong knowledge of psychiatric diagnoses, psychopharmacology, medication interactions, laboratory monitoring, risk assessment, and treatment planning. Knowledge of HIPAA, informed consent, mandated reporting, professional ethics, and clinical-documentation requirements. Fluency in Spanish, Portuguese, Haitian Creole, Cape Verdean Creole, or another language is highly valued but not required. Licensure or eligibility for licensure in Massachusetts and/or Connecticut is a plus.
