Description u nCareSouth Carolina's ROADS mobile initiative (Reaching Out and Delivering Services) offers comprehensive, remote medical care to the communities we serve using fully equipped medical exam units staffed with a nurse practitioner, LPN, and medical assistant. Because many students in rural communities don't have the opportunity to visit a doctor's office regularly, CareSouth Carolina brings the care to them. The ROADS mobile units provide care to thousands of students in the Pee Dee every year in over 57 different schools. When school is not in session, the Family Nurse Practitioner/Physician Assistant will work in one of the CareSouth medical offices. The ROADS Family Nurse Practitioner/Physician Assistant provides a variety of direct patient care services, including assessment, diagnosis, treatment, and evaluation/monitoring for patients of all ages, birth to death. The FNP/PA may also perform specialty services after training, credentialing, and certification. The FNP/PA collaborates with clinical and other staff to achieve improved patient care & outcomes during the medical visit. It will require constant collaboration and constant efforts to improve the patient experience by providing high-quality, patient-centered care. This will involve participation in training, team meetings, and huddles to facilitate the delivery of patient-centered care. This will also involve close collaboration with facilities outside of CSC for referral services when patients need these services.
