Job Description

We are seeking a dedicated Nurse Practitioner or Physician Assistant to join our Family Medicine team as a floating provider, supporting outpatient clinics across Abbeville, Greenwood, Laurens, McCormick, and Saluda counties in South Carolina. This role is ideal for a compassionate, adaptable clinician committed to patient-centered care, prevention, and chronic disease management. Primary responsibilities Deliver comprehensive primary care to patients across the lifespan with an emphasis on wellness, prevention, and evidence-based chronic disease management. Travel regularly to assigned family medicine sites within Abbeville, Greenwood, Laurens, McCormick, and Saluda counties. Collaborate with physicians, nurses, and interdisciplinary team members to coordinate care and ensure smooth transitions. Provide patient and family education on diagnoses, treatment plans, medications, and self-care. Accurately document patient encounters and maintain timely electronic health record entries. Qualifications Graduate of an accredited Nurse Practitioner (APRN/NP) or Physician Assistant program. Minimum of 2 years' experience in primary care or family medicine. Active, unrestricted South Carolina license (or eligibility to obtain). Certification or eligibility for certification in your specialty (FNP, PA-C).
